Our mission is to provide education and guidance around topics related to aging, such as planning for care needs and navigating the healthcare system. Tara Chapin is a licensed social worker with over 17 years of experience working in healthcare. She has worked in hospitals, nursing facilities, and doctor’s offices. She is a Certified Dementia Practitioner and a mobile Notary Public in the state of Ohio. Tara saw a gap in the ability of the healthcare industry to provide individualized support to seniors who have changing needs as they age. She was inspired to open her own consulting agency to help families understand their choices and make both short and long term care plans. She has extensive experience working with seniors and adults with disabilities.
